Important Notice for Students: 1098-T Form Request

If you have paid more than $1,000 for the year 2025 and wish to receive a 1098-T form, please respond to teacher@iiomonline.com with the following information:

• Your full legal name
• Your address
• Your social security number

Deadline for submissions: January 10, 2026.

Please be aware that after this deadline, there will be a $75.00 fee to receive the form.
